ROY, Utah — A Roy high school teacher has been placed on administrative leave after handing students a survey that asked them about sex and drug use.

According to a parent who posted the survey on social media, the teacher told students to fill it out, with their names on it, and to score themselves for a grade.

The final scores ranked students from “a nerd ” to “hopeless and condemned.”

The Weber School District released a statement saying in part:

“Although we strive for a standard of perfection in teaching, occasionally mistakes are made. In such cases, it will be dealt with appropriately, always with the best interests of our students in mind.”

The school also says the survey will not be used in the future and the teacher is under investigation.
